For your convenience we accept the following forms of payment; cash, check, Visa, MasterCard, American Express and Discover Card.

Most billing questions are best handled by your insurance company. By the time you get your statement , we have done our best to resolve any billing issues. If you have any questions regarding your bill, please contact your insurance company first. However, if you have already spoken to your insurance representative and still need clarification, please email our biller, Tristi, at tristi.hicks@gmail.com.  Please allow 24 business hours for her to respond.

Unfortunately we do not accept any AHCCCS, Medicaid, or most Medicare Complete/Advantage Plans. We apologize for any inconvenience.  We are unable to see you at all if you are on any AHCCCS plans.....even if it is secondary or you pay cash.
